If you are acting on behalf of a client, please complete this section.

Your Authority to Act on behalf of a client Documents

Please send us an email to recieve a secure link to attach copies of any documentation confirming your authority to act on behalf of the client.

Examples of authorisation include, but are not exclusive to:

Client Consent

  • Written consent signed by the client authorising you to communicate or act on their behalf

  • Release of Information / Authority to Share Information form

Legal Authority

  • Power of Attorney documentation

  • Enduring Power of Attorney (EPA)

  • Guardianship Order issued by the relevant tribunal or court

Court Authority

  • Court order granting authority to act for the client

  • Parenting Order or Family Court Order (if acting for a child)

  • Violence Restraining Order or Family Violence Order documentation where relevant

For Children or Young People

  • Birth certificate confirming parentage

  • Parenting or guardianship documentation

  • Foster care or child protection authorisation

Professional Authority

  • Letter of engagement from a law firm

  • Formal referral letter from a GP, psychologist, social worker, or other professional

  • Case management authority from an organisation or government department

Other

  • Any written documentation demonstrating the client has authorised you to communicate or act on their behalf.
